Your child has a doctor's appointment but you have to work. Your mother needs to pick up important documents on your behalf. You're traveling and someone needs to access your information. For all these situations, there is an authorization letter.

What is an Authorization Letter?

It's a legal document where you authorize another person to do something specific on your behalf. Unlike a power of attorney (which can be very broad), an authorization letter is more specific and temporary.

When Do You Need an Authorization Letter?

For Minors:

•Travel with one parent or without both parents
•Medical authorization when you're unavailable
•School pickups and decisions

For Personal Matters:

•Bank account access (for specific purposes)
•Document pickup at government offices
•Medical information access (HIPAA)

For Business:

•Merchandise reception
•Vendor representation for specific transactions

What MUST an Effective Authorization Letter Include

1.Your complete legal information
2.Authorized person's information and relationship to you
3.Clear, specific description of the authorization
4.Institution or place where it applies (if applicable)
5.Start date and expiration date
6.Your signature (and notarization if required)

When Do You Need to Notarize?

Notarization NOT usually required: School pickups, package retrieval, internal business authorizations Notarization REQUIRED: Minors traveling without both parents, HIPAA medical access, bank authorizations, when the institution expressly requests it
